We know that a student-centered, individually designed approach helps young people with disabilities transition seamlessly into a life of economic independence and meaningful participation in the community.

For the Secondary Transition Imperative — in partnership with national experts and organizations — we provide an implementation framework of technical assistance, self-assessment, and support that mirrors current special education accountability and compliance under state and federal law.

Through this framework, our goal as educational leaders is to implement a seamless and comprehensive statewide system of coordinated services for children with disabilities and their families in homes and communities to narrow the school readiness gap.
